UrlRewrite acting strange

I run the webpage on IIS 7 in integrated mode. I've cheked all the settings in the web config file.

 http://development.se/startsidan/?id=3 works

But this dont work
http://development.se/startsidan/

The errormessage that is logged.
Url http://development.se/startsidan/ is not recognizeable by UrlRewrite, returing it unmodified.

Any Ideas?
