Notification tells user replied to your comment, even though user commented on action

Description

_emphasized text_Precondition: ItemA belongs to a project and has the approval sequence that does not require comment on decline/approve.

Steps to reproduce

1. UserA sets 'Ready for review' the content.
2. UserB approves/declines the content.
==> The activities "The content was approved/declined by <UserB> <time>" are displayed in the Project Overview.
3. UserA comments on this activity.

Expected: The notification tells "UserA commented on your action 'The content was approved/declined'".

Actual: UserB receives a notification telling that "UserA replied to your comment "
