For 6 R2 I think you havea a chance to find out what's going on by browsing the frame source. Also look at the property settings for XhtmlString and find out which plugins are active and if you have own plugins that "does something" regarding the toolbars or TinyMCE instance.

I have a customer that occasionally get double button bars in tinymce. Like it's initialized twice or some such.
I've been able to get around the issue with ctrl+f5, but the customer claims it doesn't always work for them.
Any pointer on how to trouble shoot?
