Epilink sometimes disappear

Sometimes the Epilink/add/edit links button in TinyMCE disappears for the editors, and also sometimes me. It reappears if you do refresh/Ctrl+F5.

Since this is a EPiServer addon to the TinyMCE module, I would think the problem is related to EPiServer and not the actual TinyMCE part.

Does anybody else have this problem? The code is up-to-date with the last patch/fixpack from episerver.
