Regarding uploading video file and their thumbnail images in File Manager

Hi Team,
I would like to upload video file along with its category and Thumbnail image that will shown up in Video Gallery aspx page. I have added new File summry.config file for this.
1. But how to add 2 FileUplaod controls in Xform (one for video and other for thumbnail image) and how to access this data on page for various video files.
2. Is it possible to add dropdown list in Xforms whose values can be filled from appsetting defined in web.config
