I have implemented Google map as Dynamic content but i want to implement Google map like this

Ok, I guess in this case you have to get sources of epicode module and rebuild/adapt it for your CMS version.
Another option is to modify Google Maps dynamic content from Alloy Tech to be able to put several places on map and define different marker icons.

Yes I downloaded and installed the module but i am not getting any output shown in screen cast

No, I didn't try to use this package. It looks like module package installation is supported only on EPiServer CMS 5 R1. Also as I can see it uses deprecated Google Maps API v.2
