Subscription e-mails are not being sent

I'm having the following problem with subscriptions on an intranet where we are running EPiServer 4.41:
E-mails are sent when a subscriber registers, however no e-mails are sent to subscribers when new pages are published. The same templates work fine in the development installation, and we are using the same SMTP server in both cases.
Where do we look for the solution to our problem? Any ideas?

A clarification regarding the problem:
Subscription e-mails were sent when the scheduled job ran at one time during the last 12 hours, only it is scheduled to run once every hour. However, no e-mails are sent when the "Start manually" button is pressed, although the message "The job started without any problems" is displayed.
Another observation is that once when the job ran it failed, with the following message:
Exception has been thrown by the target of an invocation. [Access was denied to page 382]
The page in question (382) does not have access rights that differ from other pages that can be subscribed to.
Could it be a configuration problem outside of EPiServer? Something to do with access rights?
