Include Approver's name in Approved message

Description

Currently, we only show the approver's name if there is a comment present on the approval. However, this requires "Comment on Approval" to be turned on. The approver's name should always be shown.

Steps to reproduce:
1. Create approval sequence for a page with "Comment on Approval" off
2. Modify the page and set it to "Ready for Review"
3. Approve the change

Expected:
Notification and email say Approved by "Approver's Name" and Ready to Publish

Actual:
Notification and email say Approved and Ready to Publish
