The Quicksilver sample site page https://github.com/episerver/Quicksilver/blob/master/Sources/EPiServer.Reference.Commerce.Manager/Apps/Shell/Pages/Login.aspx has 

Inherits="Mediachase.Commerce.Manager.Login"

but that should be 

Inherits="EPiServer.Reference.Commerce.Manager.Login"
