CSR UI: Prepopulate fields on the Add new address screen

Description

When I click Accept Payment > Billing Address > Add new address, I see fields for first and last name. Please retrieve the first and last names from the Customer name field and fill them into these fields. The values should be editable because sometimes you're shipping an item to someone else. But in most cases, populating those fields with previously-collected customer information would save a step.

Also, once I enter a zip code and country, you should be able to populate the State field.
